Role Description

The Full Time Care Manager will be responsible for performing assessments, creating care plans, managing care, and coordinating healthcare services while advocating for clients and their families. The Care Manager will work with families, healthcare providers, and other professionals to ensure quality care and an optimal life for those they love. The position requires a team-minded individual who will collaborate with Inspire Care team members to attain client-centered care management goals. A proactive individual who is positive, a good listener, outgoing, with proven communication skills (written and verbal) is a must.

Duties & Responsibilities:

  • Perform client assessments to identify needs and determine the appropriate care plan
  • Provide Care Management services wherever the client lives or is receiving care (home, rehab, skilled nursing, assisted living, ect.)
  • Interact with client/family on a regular basis, often providing education and emotional support
  • Create and assist in coordination and implementation of the care plan
  • Maintain ongoing communication and collaboration with health care providers to advocate for client needs
  • Provide follow up home visits and on-going supervision
  • Complete accurate, concise and timely documentation
  • Attend regular staff meetings and required training

Qualifications

1. Minimum Education/Certification: Associates/Bachelors degree in nursing (RN or LPN), or Bachelors/Masters degree in social work, physical therapy, occupational therapy

2. Minimum three (3) years of care management experience (preferably with elder population)

3. Care Management Certification (CCM, CMC, C-ASWCM, or C-SWCM) is preferred

4. Valid NYS Driver's license with current auto insurance and use of private vehicle
