Department BSD MED - Infectious Diseases - Chicago Center HIV Elimination - Health Care About the Department The Chicago Center of HIV Elimination, embedded within the University of Chicago's main campus in Hyde Park, is located in Chicago's HIV epicenter. Of the 20 neighborhoods in the city with high annual HIV diagnosis rates (greater than 40 per 100,000), 13 of them are no more than eight miles from the University of Chicago. This area includes 4 of the 6 communities with the highest annual diagnosis rates in the city (Washington Park, West Englewood, Greater Grand Crossing and South Shore). Our position within the hardest hit neighborhoods in Chicago provides unique opportunities to advance HIV testing and prevention interventions locally, providing tangible results to those most affected and to improve the lives of those living with and without HIV infection. CCHE seeks to eliminate new HIV transmission events over the next 30 years (from 2011 to 2041) by using network science to target and integrate prevention as well as create structural and community-specific interventions. Job Summary In this role, the Assistant Care Manager (ACM) is responsible for completing daily or shift specific quality audits, facilitating monthly team meetings, and directing delivery of quality, patient-centered care for individuals seeking linkage to care and supportive services. This is in collaboration with and under the supervision of the Director of Clinical Engagement (DCE). In the absence of the DCE, the ACM assumes responsibility for the outreach unit. This position is grant funded. Responsibilities Responsible for training and supporting the Linkage to Care and Outreach team members. Training consists of completing the HIV testing and counseling training, team trainings on updated protocols and services (i.e. Disease Intervention Specialist protocols), and CCHE-wide trainings that inform staff of outreach policies and procedures, basic STI and HIV knowledge, etc. Complete regular audits ensuring documentation is up-to-date for the linkage to care and outreach team. Provide task management to the linkage to care and outreach team and offer weekly supervision meetings. Provide linkage to care services themselves, HIV result notification and linkage to care services for treatment and PrEP services, and HCV result notification and linkage. Responsible following the fidelity monitoring protocols in data collection and documentation in Apricot. Lastly, the ACM will have experience and be further trained in crisis intervention in order to support staff when a crisis occurs. Uses knowledge of clinical studies to coordinate the collection of analyzable clinical research data and/or samples. Performs various administrative and operational tasks under moderate supervision. Acts as a resource on the basics of clinical research and related aspects of clinical study for other clinical research staff. Performs other related work as needed. Minimum Qualifications Education: Minimum requirements include vocational training, apprenticeships or the equivalent experience in related field (not typically required to have a four-year degree). --- Work Experience: Minimum requirements include knowledge and skills developed through 5-7 years of work experience in a related job discipline. --- Certifications: --- Preferred Qualifications Education: Bachelor's degree in a relevant field. Experience: Experience working with and/or providing care to LGBT populations, Black and Latino community members. Experience providing services for people with HIV and/or HCV. Preferred Competencies Demonstrated effective leadership and teaching skills. Strong organizational skills. Strong knowledge of Chicago social service resources. Strong knowledge of program subject matter. Strong interpersonal skills and the ability to work both independently and as part of a team. Flexibility. Creativity. Ability and/or willingness to provide HIV/STI treatment, education, and prevention in non-traditional settings. Proficiency in the use of Microsoft Applications, including but not limited to Outlook, Excel, and Word. Demonstrated written and verbal communication skills and strong analytical skills. Ability to work on multiple projects.
