About the Role:The Agency on Aging of South Central CT is seeking a highly motivated and compassionate Care Manager to join our team in North Haven, CT. As a Care Manager, you will be responsible for ensuring the well-being of our older adult clients in a home care setting. Your major end result will be to provide exceptional social services to our clients, including discharge planning, mental health support, and long-term care management. You will work closely with our clients and their families to develop personalized care plans that meet their unique needs and preferences. Your role will be critical in ensuring our clients receive the highest quality of care possible.Minimum Qualifications:Bachelor's degree in Human Services or related field2+ years of experience in case management or related fieldKnowledge of Medicaid regulations and requirementsExcellent communication and interpersonal skillsPatience and compassion for older adultsPreferred Qualifications:Master's degree in Social Work or related fieldExperience working with older adults in a home care settingCertification in case management or related fieldResponsibilities:Develop and implement personalized care plans for our clients in a home care settingProvide exceptional social services to our clients, including discharge planning, mental health support, and long-term care managementCollaborate with clients and their families to ensure that their needs and preferences are metMaintain accurate and up-to-date client records and documentationCommunicate regularly with healthcare providers and other members of the care team to ensure continuity of careSkills:As a Care Manager, you will use your exceptional communication and interpersonal skills to develop strong relationships with our clients and their families.
