Position Overview
Come be a part of our team in the role of Universal Care Manager for our Community First Choice (CFC) Program, a crucial position that significantly impacts the lives we touch. As a symbol of compassion and commitment, you will play a key role in organizing and overseeing personalized services, guaranteeing that our clients enjoy a life of independence and dignity within the comfort of their communities.
Key Responsibilities
- Perform detailed assessments to evaluate the needs and preferences of our participants.
- Craft and execute tailored care plans in collaboration with participants, their families, and other caregivers.
- Oversee the provision of services, guaranteeing alignment with the goals and needs of our clients.
- Offer continuous support and advocacy for our participants, proactively addressing concerns.
- Keep meticulous records of all services and interactions, ensuring information is current and accurate.
- Work in partnership with community resources and service providers to fortify the support network for our clients.
- Engage in ongoing educational and professional development opportunities to remain at the forefront of care management practices.
Qualifications
- Bachelors degree in Social Work, Nursing, or a related discipline.
- A minimum of 2 years experience in care management or a similar role.
- Deep understanding of the unique needs and challenges confronting older adults and individuals with disabilities.
- Exceptional skills in communication, organization, and problem-solving.
- Self-motivated with the ability to excel independently and as a team member.
- Competency with electronic health records and relevant technological tools.
