About the Role:

The Agency on Aging of South Central CT is seeking a highly motivated and compassionate Care Manager to join our team in North Haven, CT. As a Care Manager, you will be responsible for ensuring the well-being of our older adult clients in a home care setting. Your major end result will be to provide exceptional social services to our clients, including discharge planning, mental health support, and long-term care management. You will work closely with our clients and their families to develop personalized care plans that meet their unique needs and preferences. Your role will be critical in ensuring our clients receive the highest quality of care possible.

Minimum Qualifications:

  • Bachelor's degree in Human Services or related field
  • 2+ years of experience in case management or related field
  • Knowledge of Medicaid regulations and requirements
  • Excellent communication and interpersonal skills
  • Patience and compassion for older adults

Preferred Qualifications:

  • Master's degree in Social Work or related field
  • Experience working with older adults in a home care setting
  • Certification in case management or related field

Responsibilities:

  • Develop and implement personalized care plans for our clients in a home care setting
  • Provide exceptional social services to our clients, including discharge planning, mental health support, and long-term care management
  • Collaborate with clients and their families to ensure that their needs and preferences are met
  • Maintain accurate and up-to-date client records and documentation
  • Communicate regularly with healthcare providers and other members of the care team to ensure continuity of care

Skills:

As a Care Manager, you will use your exceptional communication and interpersonal skills to develop strong relationships with our clients and their families. You will also use your knowledge of Medicaid regulations and requirements to ensure our clients receive the care they need. Your patience and compassion for older adults will be critical in providing exceptional social services, including discharge planning, mental health support, and long-term care management. Additionally, your skills in case management and long-term care management will be essential in developing and implementing personalized care plans for our clients.
