Description of Role: Supervises a team of Care Managers and clerical personnel. This position involves administrative oversight, clinical supervision, management of resource utilization, service planning, team productivity and deliverables and regulatory compliance. Reports to the Assistant Director or Director of Long-Term Care Options.RequirementsRequired Education and Experience:
  • Bachelor's degree in social sciences, behavioral sciences, human services, or a related field, or equivalent certification.
  • Three years of combined care management and supervisory experience, including one or more years of supervisory experience (preferred).
  • Three years of combined care management and supervisory experience, including at least one year of supervisory experience, MSW preferred in lieu of supervisory experience, preferably in a social service setting.
Job Responsibilities:a. Under the supervision of the Assistant Director or Director, carries out all such activities as assigned to meet the goals of the LTCO Department within the context of the larger agency mission.b. Supervises/coordinates the efforts of a care manager team to meet the community-based, long-term care needs of people with disabilities and chronically impaired older adults within the OPTIONS program.c. Supervises the assigned team to meet productivity and care management activity standards.d. Assists the OPTIONS Management Team with planning/developing initiatives relative to the care management process/function. Makes recommendations for continuous improvements in department policies and procedures.e. Supervises and monitors the performance of staff through individual supervisory conferences, team meetings, case reviews, staff appraisals and other approaches as needed: maintains appropriate supervision records. f. Reviews staff clinical and administrative documentation in Oracle and SAMS to maintain quality, accuracy, and timeliness.g. Remains current with system changes/upgrades through ongoing computer training and orientation.h. Assures that staff is in full compliance with applicable state standards and regulations, including OPTIONS and CHC/Aging Waiver requirements. Utilizes management reports to ensure team compliance with policies and regulations.i. Works with the OPTIONS Management Team to identify on-going training needs for staff in the area of health care, social service delivery, and care management and provides or arranges for training.j. Participates in team-based case audits and reviews and communicates findings for staff and program development purposes. Participates in department-based peer review process.k. Monitors team's assistance to participants in the MA Application Enrollment process and monitors participant conversion process to CHC (Waiver)l. Identifies problems with providers or barriers in service delivery systems and participates in resolution interventions where necessary.m. Assists with hiring of care management and clerical support staff in conjunction with Human Resources. n. Works closely and collaboratively with other PCA departments and staff to address participant needs.o. Meets various reporting requirements as required or requested by the Director of LTCO.p. Provides back up to Care Managers for caseload management during the workday when needed.q. Assists in providing coverage for vacant supervisory positions.r. Contributes to upholding a highly supportive team culture. s.
