Summary: The Care Navigator Supervisor is responsible for providing leadership and oversight to the Care Navigation team. This role involves assessing individuals' needs, connecting them with appropriate mental health and substance use disorder services, and providing ongoing support and follow-up. Primary Duties:
- Respond to service requests and schedule assessments.
- Conduct comprehensive assessments to determine appropriate levels of care.
- Identify and connect individuals with suitable service providers.
- Provide ongoing follow-up support and address any barriers to care.
- Develop and maintain relationships with community stakeholders.
- Review and submit monthly and annual reports.
- Document all interactions and meetings.
- Participate in professional development opportunities.
- Supervise and evaluate direct reports.
- Respond to service requests within one business day.
- Complete assessments within seven days.
- Review assessment results with individuals within seven days.
- Complete all follow-ups within established timeframes.
- Provide bi-weekly supervision and annual performance reviews.
- Meet all HopeLink compliance and training requirements.
Qualifications:
- Bachelor's degree and QMHP C and/or A required (Master's degree and licensed or licensed eligible preferred)
- Knowledge of Fairfax County's mental health and substance use disorder services
- Bilingual a must (English and Spanish, Vietnamese, or Arabic)
- Strong judgment and decision-making skills
- Excellent oral and written communication skills
- Ability to work independently and collaboratively
- Experience with electronic health records
- Commitment to providing culturally and linguistically appropriate services
- Adherence to program policies and procedures

- $66,000- $70,000 - Estimated salary depending on related experience
