- Answer potential client inquiries, perform in home assessments and conduct home visits to existing clients to ensure excellent care is provided and needs are met.
- Responsible for creating and updating care plan for clients. Coordinating with caregivers and family members while anticipating needs, solving challenges and addressing concerns that may arise between clients and caregivers.
- Work closely with our scheduling coordinator in maintaining client schedules to ensure the client and caregiver are matched based on preferences & qualifications.
- Assist with introductions and training for family members and clients as well as caregivers.
- Follow all office communication and documentation protocols and maintain confidentiality of all information pertaining to clients and employees.
- Phone support.
- Perform ad hoc tasks that may be assigned.
- Reach out to facilities, doctor's office, senior center etc. to establish referral sources.
- Develop and maintain relationships with potential referral sources, including local hospitals, physicians, and community organizations.
- Conduct outreach activities, including presentations and attending community events to increase brand awareness and client base.
- Coordinate client care with caregivers and healthcare professionals to ensure seamless service delivery.
- Assess client needs and create customized care plans in collaboration with healthcare professionals.
- Monitor and update client care plans as needed.
- Provide support and guidance to clients and their families.
- Ensure compliance with all federal, state, and local regulations regarding home health care.
Requirements
- High School graduate or equivalent. Bachelor's degree preferred.
- Minimum of one to three years of experience in a similar position strongly preferred.
- Positive, outgoing, detail oriented, must have strong organizational skills and ability to build strong relationships with clients and caregivers.
- Exceptional customer service, ability to problem solve and work in challenging situations and communicate effectively with concerned parties.
- Ability to communicate clearly, both orally and in writing.
- Knowledge of WellSky system preferred.
- Willingness to spend extended periods in the field with driving as a requirement.
- Ability to pass background checks, possess 2 valid ID's (passport, driver's license, or social security card), complete home care aide registration
Salary Description 22-27/Hour
