Reports To: House Manager
General Description:
Under the supervision of the house Manager, provides primary care and assistance to residentswith dementia. Interacts and engages residents throughout the day to provide a person-centered care environment that is safe, clean, and/or meaningful for each resident. Assist residents with all facets of ADL's as well as lead residents in meaningful and fun activities, providing assistance to residents as needed throughout the day. Understands that all behaviors have meaning and must interact with residents in a positive and creative manner to maximize their comfort and safety. The CP will comply with Residents' Rights at all times while working or on the grounds of Dolan.
Every CP is a universal worker. A universal worker will do everything needed including cooking, cleaning, laundry, group and individual activities, passing medications, bathing, toileting, grooming, etc. It is also vital to do rounds regularly to ensure the safety of each resident.
Personal Care:
Perform basic nursing/personal care services to residents including but not limited to bathing, shampooing, grooming, toileting, giving hygiene and comfort measures as well as feeding and hydrating residents as necessary.Take vital signs - weights, temperature, pulse, and blood pressure.Ensure the safety and security of all residents and their belongingsMaintains the inventory of personal care and toiletry supplies and provides additional supplies from house stock as needed.Keeps the Manager informed of changes in resident condition, family concerns, and/or changes in status.Medication Administration:
Follow house procedures and state regulations in the administration and storage of medications.Notify House Manager of changes in physician orders.Receive and give a report of the resident's conditions to oncoming CP.Provide documentation in MAR clinical record as directed by House Manager.Maintain medication cart and/or storage of medications including OTC items in compliance with house practice, policy, and state regulations.General:
Practice and promote positive customer service with residents and families.Is familiar with resident's ISP and life story. Promotes engagement with residents and families in a positive and professional manner while supporting person-centered care.If there is a pet staying at the house care partners must tend to and care for them as needed i.e. feeding & watering, letting them outside, walking, etc.Makes rounds regularly to ensure resident well-being and safety.Documents in the communication book. Not for complaints or gripes of staff members, information which is pertinent to residents condition, family or facility.Gossiping or other forms of communication which may be construed to be inappropriate language either verbal or written is prohibited.Attends all staff training sessions.Assist House Manager in admissions, transfers and discharges as well as other duties assigned by House Manager.Reports all incidents/accidents and or injury to residents, employees, or visitors by completion of incident report. A verbal report to immediate supervisor prior to or at the end of each shift is also required.In the event the employee would be asked to use their vehicle or ride in a vehicle in connection with their position seat belts are to worn at all times while riding or driving.Illegal drug free, smoke and alcohol free while on premises and duty.Cell phone use is prohibited during working hours.Stealing of any kind, from the resident, company, associates, and or visitors is strictly prohibited.All facilities are weapon free zones, include but not limited to guns, knives or any other device which may cause harm.Report any criminal felony charges (A,B,C) to the company when charged.Qualifications:
Med level 1, CMT or C.NA (must complete Med Level 1 class within 1 year)First aid Certified, CPR CertifiedAbility to follow written and verbal instructions in English.Demonstrate a willingness to work with geriatric dementia residents.Positive work experience within a geriatric dementia environment.Physically able to lift 50 lbs or greater, stoop, bend, and squat.
