PHC is a part of Dignity Health Central Coast's award-winning network of outstanding hospitals imaging centers laboratories and post-acute services.ResponsibilitiesIn keeping with the Pacific Central Coast Health Centers health care philosophy, the Care Team Access APP is credentialed with advanced training in areas of primary health care for adult and pediatric patients, including, but not limited to: taking health history, performing physical examination and psychosocial assessments, management of acute and chronic medical conditions. Provides clinical administrative services supporting Physicians in the clinic locationQualificationsMinimum of 5 years experience as a PAPA State Certification.
