We are currently recruiting for a Care Team Associate (CNA) for our Stamford, CT site.
POSITION SUMMARY
The Care Team Associate is a Certified Nursing Assistant or a Medical Assistant who is a member of the patient centered medical home (PCMH) care team responsible for working with providers, nurses, and other front office staff to ensure the complete and timely patient flow process. This individual exhibits sensitivity to the needs of a diverse population and must maintain strict confidentiality.
ESSENTIAL FUNCTIONS & RESPONSBILITIES
* Greet patients in accordance with established procedures.
* Relay a variety of information to the patient regarding preparation for various types of examinations and educational information; i.e. how to take temperatures, home safety, etc.
* Administer health questionnaire, takes and records vital signs (temperature, pulse, respirations, blood pressure, growth parameters (height, weight, head circumference).
* Perform basic lab procedures: i.e. hemoglobin, pregnancy test, glucose monitoring, urine test, EKG, throat cultures, and any other measurements of patients as directed.
* Adhere to infection control and safety guidelines, and confidentiality and privacy policies.
* Maintain standard exam room inventories for medical and clerical supplies as needed.
* Assist professional staff during physical examinations.
* Adhere to infection control and safety guidelines, and confidentiality and privacy policies.
* Support the needs of the unit ensuring compliance to all regulatory requirements and expectations.
* Provide Spanish / English translation as needed for patients and primary care providers.
* Clean, restocks, and checks for expiration dates on medical supplies in assigned examination rooms according to the established exam room inventory: checked on a weekly basis.
* Maintain competency in the use of clinical equipment and procedure.
* Participate in clinical team meetings; pre-work for clinical visits / huddles in preparation for patient visits and to support teamwork.
* Maintain an organized, clean, efficient and confidential work area.
* Perform other related duties as required.
JOB QUALIFICATIONS/REQUIREMENTS
EDUCATION: High School diploma or GED, attended a C.N.A. Training Program & has certification from the State of CT / Public Health Dept.
EXPERIENCE: Experience in a clinical setting. Intimate knowledge of language and culture of a sizeable segment of community served by the Health Center.
LANGUAGE SKILLS: Bilingual: Spanish-English helpful
REASONING ABILITY: Secures guidance and support from the licensed nursing staff or primary care providers regarding the delivery of patient care.
LICENSURE / CERTIFICATION: C.N.A. certification from State of CT DPH Registry, CPR certification required upon hire and recertification bi-annually.
Certification from the American Association of Medical Assistants or in process of acquiring certification, a plus. CPR certification required upon hire and recertification bi-annually.
