Care Team Nurse

As a Care Team Nurse you will provide professional nursing care for health center patients following established standards, practices, and protocols and uphold ODHS Mission, Vision, and Values.
Responsibilities (How this position enhances our Mission, Vision, and Values)
Mission - Improve the Quality of Life of Others:
* Before talking to the client/patient, determine the reason for the health center visit by reviewing the chart and observing prior notes and providers orders.
* Schedules appointments for new and returning for all health-related conditions, excluding Family Planning and WIC utilizing evidence based on protocol for all ODHS patients.
* Screens all medical problem telephone calls:
* Refers as appropriate
* Consults with providers as appropriate
* Charts actions taken in patient chart
* Follows up by contacting the client/patient.
Vision- ODHS preferred partner in East Central Indiana for improving the well-being of individuals and the communities in which they live:
* Observes, records, and reports to physician/nurse practitioner/certified nurse midwife patient's condition and reaction to drugs and treatments as indicated.
* Charts pertinent health information, including significant changes.
* Reports test and examination results.
* Ensure providers notes and signatures are written for all orders, consultation, and exams.
* Administers prescribed medications and treatments in accordance with nursing standards and clinic protocols.
* Consults with other clinic disciplines to assure the client/patient's health, nutritional, and environmental/social needs are addressed. Refers, as appropriate, to other health care providers, clinic disciplines, and community resources.
* Consults with Chief Medical Officer and /or Director of Clinical Operations regarding unresolved client/patient issues.

* Implements the nursing process and communicates with the physician/nurse practitioner regarding concerns and/or problems regarding the client/patient.
* Maintains and reviews patient records, charts, and other information.
* Maintains flexibility in nursing assignments to include, but not limited to, follow-up, chart audits, and on floor nursing duties when needed.
* Develops and maintains ongoing knowledge of nursing policies and procedures and standing orders and implements as appropriate.
* Provides ongoing teaching/counseling, including but not limited to, health status, immunizations, preventative health and health dangers, screening, laboratory test, personal hygiene, diet, birth controls, physical exams, and available community resources.
* Participates in professional development activities and maintains professional affiliations.
* Attends required meetings and in-services and participates on committees as requested.
* Maintains patient confidentiality.
* Performs all other duties as assigned.
* Supports ODHS outreach events.
* Supports other departments as needed & applicable.

About you:
* Ability to react calmly in emergency situations.
* Ability to read, understand, and follow detailed written and oral communication.
* Skill in applying and modifying the principles, methods and techniques of a professional nursing to provide ongoing patient care.
* Knowledge of organizational policies, regulations, and procedures to administer patient care and common safety hazards and precautions to establish a safe work environment.
* Skill in establishing and maintaining effect working relationships with patients, medical staff, and the public.
* Skill in initiative, listening, judgment, discretion, and decision-making within the guidelines of the policies, procedures and standing orders of the Health Center.
* Skill in preparing and maintaining records, charting, and writing reports.
* Ability to prioritize work assignments, work independently, and perform duties efficiently.
* Understanding of individual's socio-economic status and life-style with respect for individual differences.
* Requires standing, walking or sitting for long periods of time. Occasional bending, stretching or lifting up to 40 pounds.
* Requires the ability to work in stressful situations.
* Requires corrected vision and hearing to normal range or use of approved adaptive equipment
Typical Working Environment: Frequent exposure to communicable diseases, toxic substance, medicinal preparations, and other conditions common to clinic environment. Tasks involve contact with blood or body substances.
Education: Minimum graduate of an accredited school of licensed practical nursing, RN preferred.
Experience: Prefer two years of professional nursing experience in clinic/office setting.
Certificate/License: Possession of an Indiana State Nurse license. CPR certification. TB certification, required to be maintained through ODHS post hire.
