How You Will Make a Difference As we expand our services in new and existing markets, we're looking for a Care Transition Coordinator who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, Called to serve patients in their home. The Care Transition Coordinator is the face of VitalCaring when interacting with referral sources. In this role, you orchestrate the perfect balance of patient care and operational efficiency, building confidence among care teams, patients and providers. As a result, your clients view VitalCaring as their preferred hospice partner that provides compassionate, personalized hospice care to their patients. As a Care Transition Coordinator, you will:
- Cultivate and nurture relationships with referral sources, patients and families
- Be a source of knowledge and information about our service offerings, the unique elements of hospice and the benefit of hospice services for patients and families facing the end of life
- Utilize available tools and market knowledge to understand market dynamics and identify productive sources of growth
- Collaborate with operational leaders to support a strong team culture, address challenges, promote accountability and drive continuous improvement
- Consistently deliver on monthly individual performance goals

Experience to Deliver on our Mission
- College degree, valid state driver's license, and reliable transportation.
- One year of hospice or healthcare sales experience.
- Knowledge of business and fiscal management, governmental regulations, and accreditation standards.
