Job Title: Care Transitions Navigator - Behavioral Health Job Description: The Care Transitions Navigator position includes hands on interaction with patients who have complex needs including but not limited to psychosocial, psychiatric, and/or substance use issues presenting to Baptist Health Richmond. Their role extends beyond medical treatment to consider the social, emotional, and environmental factors that may impact a patient's well-being.
- Provide comprehensive psychosocial consultations and support for patient and/or family.
- Provides discharge planning services in conjunction with multidisciplinary treatment team.
- Maintains up to date knowledge and database of community-based services and resources in appropriate community networks.
- Facilitating assistance with resources and referrals based upon patient needs.
- Addressing psychosocial concerns and social deterrents of health to ensure effective transition from hospital to post hospital care through planning, relaying information and facilitating referrals to community agencies.
- Acts as an advisor and resource to fellow staff members
- Maintains professionalism, participates in meetings, staffing and committees as requested.
- Maintains accountability and responsibility for tasks.
- Assists with completion of utilization review for direct admissions to Baptist facilities as needed.
Requirements: - Master's Degree in Social Work (MSW) with a current KY License as a Certified Social Worker (CSW) or current KY License as a Licensed Clinical Social Worker (LCSW) required.
- For the CSW, supervision hours must be completed within 3 years of hire, and licensure must be obtained within 8 months of completing the supervision hours.
- Master's degree in counseling with associated licensure (LPCA, LPCC) may be considered in place of the social work degree/licensure. Professionals must good standing with Kentucky Boards of Social Work and Professional Counseling
