A partner of the Optum Care network, WellMed Medical Group, is seeking an Associate Medical Director (AMD) to oversee the CareCoach Connect program in East Texas – Tyler, Longview, Nacogdoches, Lufkin. The CareCoach Connect program provides both longitudinal and episodic medical care to mostly Medicare-aged patients on a Medicare Advantage plan. That care is delivered by Advance Practice Clinicians (APCs) in close collaboration (~1:5 ratio) with AMDs. The APCs and AMDs will conduct visits in patient's homes and virtually to manage their acute and chronic conditions. This role will lead a team comprised of several physicians and their respective APC team (referred to as a POD). The AMD meets regularly with the clinical and operational leadership locally and regionally.
