Client location and schedule hours:Client located In Nashville . the schedule is M and W 9 am to 2 pm. and Sun 8a. to 1 pJob Responsibilities:Diagnoses: Stage 3 Merkle cell cancer metastasized, Lymphedema, Congestive Heart Failure, Weakness in knees (needs knee replacement).Goals & Objectives: Assist with a shower each day; into and out and with drying off and dressing. Remind and assist to take meds as scheduled, documenting dates and times taken. Keep hydrated, prepare and serve meals as desired. May need to run errands or put away grocery deliveries. Light housekeeping, laundry & bed linen changes as needed.ADL's: Mobility- Client can get himself up from a lying or seated position by himself. He is good at using rails or support bars available. This is a slow process due to his pain. He can also stand up and get onto his walker by himself and can take several steps, also independently. He needs help getting into & out of a vehicle & family is in the process of changing vehicles to make this easier.Bathing/Dressing/Grooming- Client likes to get a shower each day, preferring morning, but he is ok it's later in the day. He needs help undressing and stepping into and out of the shower. He needs help drying off. He will need help putting on a fresh depends and to bring him clean shorts/shirt as needed.Toileting: Client can walk into the bathroom on his own and sit on his built-up toilet seat, using his support bars. Once he's done, he can stand but needs help wiping with both toilet paper and wet wipes. He will need help putting on a fresh depends and to bring him clean shorts if needed.Eating: Client can feed himself and can drink whatever is brought to him, preferring to use a straw. He does need meals/snacks prepared and served to him. He likes protein drinks but told me he really has no appetite anymore, so needs to be encouraged.Communication: Client speaks clearly and can be easily understood. He has no dementia or memory loss. No hearing loss.Qualifications:Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family memberAbility to effectively communicate through written and verbal languageValid Driver's License and adequate auto insuranceConsent to criminal, elder abuse, motor vehicle and sex offender background checksConsent to drug and alcohol testingTB test
