Care Specialist (RN)
Summary:
This caregiver position aids in daily living and personal care services in assigned FFHC patients' homes in accordance with health assessment plan that is aligned with the approved physician-ordered service plan.
Work Hours:
This is a full-time or part-time position which requires various fixed and flexible shifts hours for weekdays and weekends as needed per Agency client's strength.
Duties and Responsibilities:
- Provide healthcare services for patients and residents.
- Assist client with personal care including personal hygiene
- Maintain accurate patient documentation including date, time, task, care, and service.
- Maintain open communication between families and FFHC care staff (i.e., LPN, RN, STNA, Chief Nurse) regarding any medical and emotional conditions.
- Assist patient with scheduling appointments (Dr.'s, transportation) ? Assist with housekeeping, laundry, and shopping services.
- Ensure highest client safety and well-being.
- Excellent written and communication skills.
- Complete Performance Evaluation Plans, Annual Evaluations and Meet with Supervisors as directed
- Maintain confidentiality, integrity, quality care, and professionalism.
Qualifications:
- Ability to Pass Caregiver Certification and Skills Lab Training
- Up-to-date CPR/ First Aid Certification and Maintenance of Mandatory 75HRS of CEUs every 2 years.
- Must be physically and mentally fit to handle the demands of caregiving.
- Successful BCI, FBI, and drug screen.
