Sapphire Health Services is currently seeking a dedicated Caregiver to join our team and uphold our commitment to excellence.JOB SUMMARY:Working as a Caregiver in our Community Based Care Facility, you will assist residents with daily activities, promoting independence, and ensuring their safety and well-being, while also providing companionship. Caregivers work closely with the resident, their family, and the care team to provide personalized care that meets each individual unique needs and preferences. maintain the highest quality of care by ensuring our residents needs are met while providing companionship. RESPONSIBILITIES:
  • Provide assistance with activities of daily living (ADLs)
  • Monitor and report any changes in the residents physical or mental health Assists with serving resident meals
  • Maintain resident confidentiality
  • Provide companionship and build positive relationships with clients to reduce feelings of isolation or loneliness
  • Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports
  • Encourage clients to participate in activities that promote physical, mental, and emotional well-being
  • Support clients in maintaining independence in their daily routines and decision-making
  • Ensure a safe environment by identifying and addressing potential hazards in community setting
  • Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols
  • Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care
  • Communicate effectively with clients, families, and team members to ensure clarity and understanding of care plans and any changes
  • Present for staff meetings and trainings
  • Other duties as assigned
REQUIREMENTS:
  • High school diploma or equivalent required
  • Previous experience in caregiving, home care, or community-based care is preferred
  • Ability to work independently and as part of a team
  • Patience, empathy, and a genuine desire to help others
  • Good organizational and time-management skills
  • Ability to perform physical tasks such as lifting, bending, and assisting clients with mobility
  • Must be able to stand, walk, and reach for extended periods
  • Ability to pass a background check
