Our Life Sharing program aims to integrate individuals into a family setting, promoting independence, personal growth, and community involvement.Position Overview:We are seeking compassionate and dedicated individuals or families in Pennsylvania to become Life Sharing Host Home Providers. As a Life Sharing Host Home Provider, you will open your home and heart to an individual with intellectual or developmental disabilities, providing them with a nurturing and supportive environment.Responsibilities:Job Duties for a Life Sharing Host Family in Pennsylvania
- Ensure the home is clean, safe, and accessible.
- Create a welcoming and inclusive atmosphere.
- Provide a private and comfortable living space for the individual.
- Assist with personal care, including bathing, dressing, grooming, and toileting as needed.
- Prepare nutritious meals and assist with feeding if necessary.
- Help with medication administration and management.
- Support the individual in maintaining personal hygiene and health.
- Encourage the development of life skills, such as cooking, cleaning, and money management.
- Support the individual in participating in community events, recreational activities, and social gatherings.
- Accompany the individual to appointments, outings, and activities as needed.
- Assist with educational or vocational support
- Monitor the individual's health and well-being, reporting any concerns to healthcare professionals.
- Arrange and accompany the individual to medical, dental, and therapeutic appointments.
- Maintain open and effective communication with the individual's support team, including case managers, therapists, and family members.
- Complete required documentation and reports in a timely manner.
- Implement and follow individualized support plans, including behavior intervention plans.
- Participate in initial and ongoing training sessions required by the organization and state regulations.
- Stay informed about best practices in caregiving and support for individuals with disabilities.
- Attend meetings, workshops, and conferences as required.
- Follow all organizational policies, procedures, and guidelines.
- Ensure compliance with state and federal regulations related to Life Sharing programs.
- Maintain confidentiality and respect the privacy of the individual and their family.
- Respect and accommodate the individual's cultural, religious, and personal preferences.
- Create an inclusive environment that honors diversity and individual differences.
Qualifications:
- Must be a resident of Pennsylvania.
- High school diploma or equivalent.
- Experience in caregiving or working with individuals with disabilities is preferred but not required.
- Strong interpersonal and communication skills.
- Ability to provide a stable and nurturing home environment.
- Must pass background checks and meet health and safety requirements.
- Valid driver's license and access to reliable transportation.
