```Responsibilities```- Provide direct patient care in accordance with the individualized care plan- Monitor and assess patients' health conditions and report any changes to the team- Assist with activities of daily living, including bathing, dressing, and grooming- Prepare nutritious meals- Perform light housekeeping tasks to maintain a clean and safe environment- Provide emotional support and companionship to patients- Follow HIPAA guidelines to ensure patient privacy and confidentiality
