Services include assessments, case management, counseling, outreach, group training, education, and facilitation of support groups. The specialist participates as a member of the Older Adult Services team.Primary ResponsibilitiesParticipate as a member of the Older Adult Services Team promoting the independence and quality of life of older persons, caregivers of older persons, and grandparents raising grandchildren. Also provide aging network staff, care coordinators, and community members with training and educational opportunities, emotional, financial, and physical resources to support the work of family caregivers.Within this context, the Caregiver Specialist will be responsible for both ongoing clinical and administrative duties.Promote the CRC in the community to identify new participants that can benefit from the program.Will adhere to program guidelines and budget to meet program expectations.Develop excellent knowledge of programs, benefits, and services available to older persons on a local, regional, state, and federal level.Clinical ResponsibilitiesWorks in conjunction with the Countywide Caregiver Coordinator.Performs all clinical responsibilities within the time allocated in the preferred service mix established by the funder.Completes caregiver assessments using the funder's recommended screening and assessment tool.Completes an assessment of each caregiver participant and develops a care plan utilizing required forms.Implements and monitors care plans, coordinating community resources with other Supportive services.Provides counseling for caregivers and family members, consultation to aging network staff and the community, crisis intervention, and individual caregiver training.Manages a caregiver caseload and conducts monthly follow-up calls to ensure the caregiver's needs are met.Creates and implements an outreach plan targeting all types of caregivers through activities at area businesses, schools, religious and community organizations.Create and Implement an annual educational plan to meet grant requirements.Provide at least two classes annually according to grant requirements.Provide/Facilitate monthly trainings on appropriate topics.Facilitates monthly caregiver support groups.Communicates with care coordinators regarding issues on ongoing client care.Provide support and information to division members on available resources concerning caregiver issues and services.Develops and maintains a caregiver resource library.Receive supervision and provide feedback regarding clinical and administrative issues as needed with Director.Actively participate as a member of and provides education to other members regarding caregiver issues in team meetings.Actively participate in regular performance reviews.Arranges and implements respite service for eligible clients referred by care coordination staff.Administer GAP Filling Services to caregivers with unmet needs.Administrative DutiesComplete and submit, in a timely and accurate fashion, documentation and reports as required by AgeOptions, the Department on Aging, local funders, and the Center's policies and procedures.Enter and maintain monthly data entry units in CMIS and AgingIS, in compliance with funder requirements.Collaborate with Billing department to maintain, track, and record Respite services and spending on designated spreadsheet.Maintain complete and timely documentation for all open caregiver clients.Attend quarterly meetings called by AgeOptions.Participate in orientation and in-service programs as required by Age Options, the Department on Aging, local funders, and the Center's policies and procedures.Use software for referral and tracking as required by funder.Assure that information is available on the agency's web page and advertises e-mail address so potential clients can receive information.Position RequirementsMSW preferred from an accredited college of Social Work or BA/BS BSW, RN degree in a closely related discipline.Well-developed clinical skills, including counseling and support group facilitation experience.At least two years experience in social service provision to older persons.ScheduleEvening and weekend hours as required.Kenneth Young Center is an Equal Opportunity Employer.#J-18808-Ljbffr
