Community Living Connections (CLC) is seeking passionate individuals to join our team as Supported Living Staff, providing essential, person-centered support to individuals with developmental disabilities. Whether you're an experienced caregiver or new to the field, we will provide the training you need to succeed!
Location: Madison West Side
PT w/Two Shifts Available:
- 12 PM to 8 PM Weekends
- 3 PM to 5 PM Mon-Thurs

Key Responsibilities:
- Person-Centered Support: Implement individualized care plans tailored to the unique needs and goals of each person you assist. This includes fostering personal growth, life skills, and community integration.
- Daily Living Assistance: Provide support with household tasks such as cooking, cleaning, and organizing, while encouraging independence in these activities.
- Health & Wellness: Help with personal care routines like hygiene, grooming, and promoting healthy living habits, ensuring a safe and clean environment.
- Community Engagement: Support individuals in exploring and utilizing community resources, attending appointments, and participating in social activities.
- Financial & Transportation Support: Help with budgeting, grocery shopping, and transportation needs based on each individual's schedule.
- Communication & Collaboration: Partner with families, managers, and external professionals to provide high-quality care.
What We're Looking For:
- A desire to support individuals with developmental disabilities.
- Patience, empathy, and excellent communication skills.
- Ability to work independently and as part of a team.
- Willingness to participate in ongoing training and professional development.
- Flexibility to work varying shifts, including weekends and evenings.
- A valid WI driver's license, access to a reliable vehicle, and proof of vehicle insurance that meets CLC's standards.
Qualifications:
- High School Diploma or equivalent.
- No prior experience necessary-we provide all required training.
- Ability to maintain a valid driver's license and meet driving record standards.
Physical Requirements:
- Ability to lift up to 40 lbs. occasionally.
- Ability to stand, walk, bend, and assist individuals with daily activities.
