Job Description:

Location: Brooklyn, NY

Pay Rate: $24.04 - $28.85/hour

Bonus: $1000 offered half distributed at 60 days, and the remaining half at the 120-day mark.

Duration: Contract-to-hire

Overview: Engages patients in the initial and ongoing treatment processes at the clinic. Responsibilities include conducting preliminary eligibility assessments, preparing intake forms, ensuring patients receive necessary services from other disciplines within the agency, and making referrals to outside resources when necessary.

Responsibilities:

  • Hold regularly scheduled sessions with patients to address their problems and psycho-social needs.
  • Serve as a Case Manager for each patient to ensure coordinated services meet assessed needs.
  • Conduct therapy groups.
  • Assist patients in identifying and defining problems.
  • Ensure all necessary forms are completed.
  • Maintain accurate and confidential patient records.
  • Attend training seminars on treatment techniques and theories, and obtain the CASAC credential.
  • Identify and record significant data in case folders regularly, and develop treatment plans for presentation to the IDT.
  • Hold individual conferences with other disciplines to facilitate effective treatment.
  • Meet weekly for clinical supervision with the Behavioral Health Supervisor.
  • Refer patients to other clinic disciplines when appropriate.
  • Meet periodically with other staff for case management and treatment planning feedback.
  • Perform forensic collection for patients and monitor treatment-related issues to update the treatment plan.
  • Provide vocational/educational assessment, counseling, and follow-up for patients not assigned to the Vocational Specialist.
  • Complete the Vocational Profile Form (HS 305) for each patient.
  • Provide counseling for vocational/educational readiness and motivation for vocational rehabilitation.
  • Refer patients to the Vocational and Educational Specialists for in-depth evaluation and counseling.
  • Provide case management, referral follow-up, and supportive counseling for patients in vocational/educational programs.
  • Meet periodically with the Vocational and Educational Specialists for case consultation and feedback.
  • Present vocational treatment objectives at each IDT.
  • Work on a rotation basis on Saturdays and select holidays as assigned.
  • Perform other duties as assigned by the manager.

Requirements:

  • CASAC (Credentialed Alcoholism and Substance Abuse Counselor) Certification required.
  • Computer literacy with knowledge of Microsoft Office applications and competency in computerized programs supporting treatment plans and case notes.
  • Excellent verbal and written communication skills.
  • Ability to participate in various committees and work well in team settings.
  • Bilingual in any language is a plus, but not required.
