Case AideSummary of the Job:The Case Aide is responsible for providing case support to case managers, serve as a resource for school and families and provide additional support for youth in assigned program. The Case Aide may travel with clients via plane or auto for reunification, and provide local transport support for family visits, court or other appointments. The Case Aide will also provide intake support when Case Managers are not available. The Case Aide functions as a member of the Multi-disciplinary Team and provides case support for Case Managers who carry primary case management responsibilities. The Case Aide should be committed to Sanctuary practice utilizing tools that reinforce our shared language to create a trauma-responsive culture.
Primary Responsibilities: Maintain client case record to include intake documentation, service plan and all other required documentation on time and accurately. Maintain case records and other required documentation to assure full compliance with agency, licensing, accreditation and ORR contract requirements. Effectively utilize computer software systems used by the agency, program and contractors to support client records and requirements. Develop and implement plan for safety and well-being of client. Assist in implementing a plan for the client's permanency, which may include providing travel support for the client during discharge to identified placement. Process intakes according to agreed procedures and standards. Provide all required documentation for assigned duties, meeting deadlines for required reports. Provide support to House Managers and Youth Care Workers, filling in as needed. Utilize personal safety plan for managing emotions and interactions with staff and clients. Maintain a supportive caseload as assigned by the agency. On-Call duties as assigned.Qualifications:
Bilingual Spanish/English required At least 21 years of age. Master's degree (from an accredited school) in social work, psychology, education counseling with two years experience in the field of childcare. Possess the ability, flexibility and maturity to render the duties and responsibilities required of this position. Problem solving ability; able to develop and implement procedures. Strong written and verbal communication skills. Proficient in agency required computer skills. Adequate training or experience to demonstrate the possessions of knowledge and skills necessary to perform the duties and tasks assigned to this position.
