Job Title: Case Manager - RN

Job Summary:

We are seeking a dedicated and experienced Case Manager - RN to coordinate and ensure the delivery of high-quality care to patients. The role involves collecting utilization management and quality assessment data, collaborating with physicians to clarify plans of care, and reviewing patients for high-risk criteria to address discharge planning needs.

Duties:

  • Monitor patients to determine timely discharge needs and conduct initial case reviews within 72 hours of admission.
  • Develop discharge plans for uncomplicated cases and make referrals to Social Workers for complex discharge planning as needed.
  • Collaborate with the healthcare team to facilitate care progression, ensuring time-oriented care plans with clearly defined objectives.
  • Conduct concurrent reviews, collect performance improvement data, and review cases for continued stay appropriateness.
  • Maintain current knowledge of field developments through professional literature, continuing education, and participation in professional organizations.

Skills:

  • Minimum three (3) years experience in direct patient care.

Education:

  • Bachelor s degree in nursing (BSN) required; Associate s Degree accepted with minimum five (5) years experience in direct patient care.
  • Master's degree in nursing preferred.

Required Certifications & Licensure:

  • Current NY State RN License required.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) required.
  • Preferred Certifications & Licensure:
  • Pediatric Advanced Life Support (PALS) and Case Manager Certification (CCM) preferred.

Position Details:

Location: Brooklyn, NY 11212

Position Type: 13 Weeks

Pay Rate: $67.00 - $71.00 Per Hour

Shift and Time: DAYS - 8A-4P - 1 SATURDAY PER MONTH REQUIRED
