Position Overview

  • Location: Newton, NJ
  • Schedule: Monday to Friday, 8:30 am to 4:30 pm (40 hours/week)
  • Work Environment: 75% fieldwork (Will visit office for supplies, documentation, IDT)
  • Pay Details: $50-53/hour; Mileage reimbursement provided

Key Responsibilities

  • Perform comprehensive physical and psychosocial assessments and follow established care plans.
  • Engage with patients and families to explain patient rights, advanced directives, and the Medicare Hospice Benefit.
  • Document patient visits, vital signs, and interventions using the NDOC EMR system.
  • Facilitate patient and family education and promote self-directed care.
  • Serve as a resource and provide direct patient care, including pronouncements and admissions.
  • Maintain infection control standards and uphold patient confidentiality.
  • Participate in interdisciplinary team (IDT) meetings and maintain productivity aligned with agency standards.

Your Expertise

  • Skills: Strong clinical knowledge, meticulous documentation, and exceptional communication.
  • Qualifications: Registered Nurse licensed in New Jersey; Diploma, Associate, or Bachelor of Science in Nursing (BSN preferred).
  • Experience: Minimum one year of clinical experience; prior hospice/home care experience is a plus.
  • Tech Savvy: Proficient in Microsoft Office and EMR systems.
