Job Description:

Key details:

Shift: 8P – 4.30p, Day Shift

Guaranteed Hours: 40 Hours in a Week

Weekly Pay Range: $2100.00 – $2130.00

Responsibilities:

  • Assess patients' medical, emotional, and social needs to create individualized care plans.
  • Coordinate and oversee care services across healthcare settings.
  • Monitor patient progress and modify care plans as needed.
  • Educate patients and families on care plans, treatments, and resources.
  • Document all case management activities accurately and timely.
  • Advocate for patient needs and facilitate resource utilization.

    • Qualifications:

    • Active RN License for FL or Compact license is required.
    • Case management experience is required
