Description:
Position Purpose:
The Case Management Nurse works with the interdisciplinary team for patients on a select unit from admission to discharge. This care will be determined by individualized plans of care, care guidelines, best practice data and critical paths.
What you will Do:
* Mobilizes resources and manages the systems to achieve outcome-based care
* Coordinate physician, patient and payer interactions to ensure appropriate level of care.
* Respond to concurrent and retrospective data analysis identifying trends in patient care for that unit's patient population.
Hours/Schedule:
.9 fte, Days.
Minimum Qualifications:
Active IA RN license
BSN preferred
Minimum of 4 years of clinical experience.
