Case Management Specialist Salvation Army, 340 14th Ave S, St. Petersburg, FL 33701, USA Req #33563 Friday, November 15, 2024 Location: The Salvation Army St Petersburg Area Command, St Petersburg, FL Position Type: Regular Full-Time at 40 hrs/wk Pay Range: $20.50/hour About this opportunity: This position is responsible for interviewing, accepting, and providing comprehensive, long-term, structured, complex case management services for an assigned caseload of clients from the community and clients participating in the Pathway of Hope program. Key responsibilities: Interviews applicants to determine eligibility for The Salvation Army services or program enrollment based on established guidelines and requirements; conduct needs assessment, obtain pertinent information, and establish program goals for eligible clients; refers applicants to other agencies, if not appropriate for The Salvation Army services or programs. Develops intake and case plans for client; instructs clients in completing necessary paperwork including goals and/or budget forms; completes orientation check-off form and obtains release of information forms when appropriate; completes social history on new clients each week. Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same. What we are looking for in you: Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field, AND Three years progressively responsible experience providing direct case management social services including accessing clients needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources, OR Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities Valid State Drivers License Must be able to pass a Level II Background screening. Physical Requirements and Working Conditions: Work is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
