As a Case Manager , you will be an essential part of an interdisciplinary team, providing care coordination for severely mentally ill adults and children with complex medical conditions and/or co-occurring substance use disorders. Your role involves advocating for clients, engaging with community agencies and healthcare providers, and ensuring access to necessary services to enhance wellness, self-management, and reduce emergency room visits or hospitalizations.

Responsibilities:

  • Offer clinical support to the team through consultation, education, and information on conditions, interventions, and resources, focusing on outcomes and best practices.
  • Conduct initial and ongoing assessments of clients to document strengths, needs, goals, and available resources.
  • Maintain organized, timely, and accurate documentation, records, and reports as per policy and procedure.
  • Provide face-to-face outreach, engagement, and service planning in the community.
  • Serve as a link to community services, including medical, behavioral, residential, entitlement, and other necessary services as per the interdisciplinary care plan.
  • Deliver crisis intervention and follow-up support.
  • Perform additional tasks and duties related to job responsibilities as assigned.

Requirements:

  • Preferred: Bachelor s Degree in Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Physical or Recreational Therapy, or related fields.
  • Required: GED.

Experience:

  • At least one (1) year of experience in human services, providing direct services to clients with severe mental illness, HIV/AIDS, or other disabilities, linking them to essential community services.
  • Ability and willingness to travel regularly, including transporting clients in an agency vehicle, using various modes of reliable and safe transportation.
  • Excellent interpersonal and time management skills are essential.
