Job Type Full-timeDescriptionGreater Nashua Mental Health is looking for a full-time ACT Case Manager to join our Assertive Community Treatment (ACT) Team! The ACT Case Manager provides services to individuals who have a severe and persistent mental illness and/or co-morbid diagnosis, including a history of substance abuse, active addiction process, and metabolic syndrome, that may impact and or complicate their overall treatment, treatment plan, and prognosis for recovery. The ACT Case Manager meets with clients within their natural environments.Duties & Responsibilities: Provides direct client services for a percentage of time per week as appropriate for position and as directed by supervisor.Reviews all cases assigned to the Case Manager to familiarize self with client needs and history; assesses the need for further evaluations and review of interim treatment plan.Provides case management services to consumers as well as FSS services to include: medication monitoring, crisis intervention, and family supports, therapeutic behavioral support, etc.Coordinates the consumer's Individual Service Plan (ISP) within the Agency and with eternal agencies; communicates, collaborates, and consults with other staff and agencies. Ensures the ISP services are carried out when the consumer allows.Serves as the consumer's advocate in obtaining needed services both within GNMH and in the community.Coordinates referrals for consumers to the appropriate levels of care in the community.Assists clients in exploring the potential for future growth opportunities.Completes all treatment plans based on consumer history of illness, assessment, and past and current function, results of evaluations, and multi-disciplinary review, by due date listed in EMR. Provides clinical input and formulation of ACT Treatment Plan, which identifies specific problems to be addressed, target goals related to problems, and clear and measurable objectives with clear time frames attached to specific service to be provided.Provides treatment to consumers and families according to the established treatment plan, utilizing approved modalities of the Agency.Assesses crises and provides crisis intervention; in coordination with the client's primary psychiatrist, develops referrals for more or less intensive services appropriate to the consumer need.Provides necessary contact and treatment after hours and on weekends, as needed or determined by the treatment plan.Provides after hours emergency consultation coverage on a rotating basis.Executes the crisis needs, as outlined in the NH Involuntary admission process and Conditional discharge revocation process.Collaborates with the client's representative payees and guardians for continuity of care, if needed. RequirementsQualifications: Bachelor's Degree in Social Work, Psychology, Human Services or related field.1-2 years of experience.Strong collaborating skills and an ability to work effectively both independently and as part of a team.Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.Our agency is a team of talented and dedicated professionals who are passionate about helping the community and each other!
