Join Our Team as a Talent Acquisition Specialist at Crystal Springs!Crystal Springs is looking for a dedicated and experienced the Case Manager who will is responsible for providing case management services to the individuals and their families; initial and on-going collateral work with licensing agencies, funding sources, referral sources, guardians, courts, and other programs; coordination of internal services that include quality assurance, conducting pre-placement interviews and supporting the intake process; coordinating and facilitating individual transitions to new homes within the program and/or to other agencies; and maintaining and/or maintaining needed resources. The Case Manager participates collaboratively with all members of the management team.Standards of Care:

  • Address the total individual, inclusive of medical, psychosocial, behavioral, and spiritual needs

  • Facilitate transitions of care, which includes a complete transfer to the next care setting provider (internal and external) that is effective, safe, timely, and complete
  • Provide professional presentation when representing the agency with external parties
  • Ensure that all family members, guardians, and visitors are welcomed and treated with dignity and respect
  • Work collaboratively with funding agencies, licensing agencies, and guardians in the best interest of the individual and alert appropriate administrator(s) in the identification of potential conflicts
  • Ensure visits, telephone, and mail contact with family or other identified supportive adults in accordance with the individual service plan and program rules and standards
  • Promote interventions that promote the development of independence and improved quality of life
  • Maintain professional boundaries and confidentiality with individuals, families, guardians, and other external stakeholders
  • Establish and maintain a positive collaborative working relationship with families/guardians, day habilitation program, residential supports, nursing department, providers and other appropriate agencies
Case Liaison:
  • Serve as the primary liaison to families/guardians, agency supporters and provider organizations
  • Communicate treatment progress, issues, concerns, and general status to families/guardians, and other relevant external case workers as needed, and minimally once every two weeks
  • Maintain routine contact with day habilitation program, residential supports, nursing department, providers and other appropriate agencies
  • Ensure adequate and on-going communication of progress, issues, concerns, and general status to all members of the individual's team
  • Provide initial response to most concerns and complaints from parents, guardians, workers, and other external stakeholders, working closely with the Supervisor II's, Residential Coordinators, and Director of Residential Services
  • Distribute Satisfaction Surveys to parents/guardians as per Crystal Springs' procedure
External Meetings and Hearings:
  • Attend relevant case and administrative reviews, court reviews and hearings, hospital discharge planning meetings, and other off-campus meetings designed to ensure clear communication and cohesive service delivery
  • Coordinate and attend pre-placement interviews to plan and prepare intake and placement for admitting individuals
  • Coordinate and attend placement interviews to plan and prepare post-discharge placement and treatment/support for discharging individuals
Management Team - Leadership:
  • Assist in the leadership of the residential team, as a member of the management team
  • Participate as a member of the management team to ensure the adequate and relevant discussion and communication of individual progress, needs, concerns, etc. to other members of the management team and to inform the team of decisions that pertain to the individuals
  • Attend all required team, management, and campus-wide meetings
  • Participate and support activities as necessary and assigned
  • Provide specialized training to supporters, supervisor I's and II's, and residential coordinators in the areas of advocacy, case management and care records maintenance
Reporting and Notification:
  • Report all therapeutic physical interventions to families and relevant external agencies
  • Review and coordinate all incident reports
  • Facilitate critical notifications and related documentation using Crystal Springs' form and protocol, in addition to systems and formats required by specific funding agencies
  • Ensure completion and appropriate documentation and filing of all paperwork required of funding agencies
  • Complete and submit Case Manager Weekly Report reflecting specified task completions
  • Compile and submit monthly Individual Critical Information report listing
  • Report all anticipated changes in funding as soon as indicated including approved 1:1 staffing to Director of Adult Services and the Chief Financial Officer
  • Facilitate critical notifications and related documentation using Crystal Springs' form and protocol, in addition to systems and formats required by specific funding agencies
Family Work: (includes substitute families / guardians, as appropriate).
  • Identify and communicate family concerns, fears, or resistance to treatment to the treatment team members, and assist in resolving related issues
  • Coordinate, document, and communicate family visits
  • Coordinate and communicate approved visitor, phone, and mail contacts
  • Distribute weekly visitation schedules
Individual Record and background information:
  • Collect and compile all pertinent reports and data on individuals on caseload as related to areas of individual's life
  • Coordinate and ensure that post-admission paperwork is completed, and that all required consent, records, and other necessary forms are signed as appropriate and filed in individual care records, and updated as per protocols
  • Communicate with external referral sources and agencies and obtain and gather all relevant and required records, tests and evaluations, reports, etc. needed for evaluation, risk assessment, review, and service planning
  • Develop individual Face Sheet, and update these as required
  • Document and maintain contact notes of significant in-person, phone, and other contacts with family members, external workers (case workers and social workers etc.), as well as individual contact, and internal communication, and ensure that these are placed in the individual care record monthly
Service Planning:
  • Oversee the development and implementation of Individual Service Plans (ISP) in collaboration with individual's personal choices and preferences, which are individually tailored to his/her strengths, needs and interests
  • Facilitate and coordinate ISP meetings
  • Coordinate and collect treatment quarterly reports and treatment goals for ISP
  • Write and distribute Individual Services Plans, following ISP meetings
  • Coordinate intake and discharge related planning
  • Communicate intake and discharge notifications and plans to relevant internal and external parties
  • Coordinate intake and discharge process, including packing of personal belongings and transportation plans
  • Participate in Individual Services Plan (ISP) meetings
General Responsibilities:
  • Attend team meetings, ISP meetings, in-service trainings, conferences and seminars as required by Director of Adult Services
  • Assist in admissions process as assigned
  • Cooperate and participate in internal investigations
  • Perform other reasonably related duty as assigned
Qualifications:
  • Bachelor's degree in related field of study
  • Minimum of two years' case management experience in Human Service Field
  • Knowledge of Department of Developmental Disabilities and Day Habilitation regulations and CARF standards preferred
