Role Description As Case Manager, you will report to the Program Manager for your assigned program track. This position is an integral part of the treatment team and functions in coordinating orientations, treatment planning and discharge planning with patients. As Case Manager, you will be communicating with referral sources and other treatment centers to ensure that patients connect to services they need within the community. Key to your success will be relationship skills, creative problem solving and organization. You will be looked to as a trusted team member and important bridge to help patients integrate smoothly into the community post-treatment. Essential Functions/Core Responsibilities Responsible for welcoming new patients into the program, assisting with completion of admission paperwork, new patient orientation and completing clinical update as needed. Responsible for providing discharge planning for patients as needed and coordinating care upon discharge, collaborating with partnering facilities and referral sources. Coordinating exit interviews and exit surveys. Responsible for coordination with outside providers within 24 hours of patient's admission and ensure ROI is signed and send contact information to therapist. Attend team meetings in which patient cases are discussed. Establishes and maintains effective communications and working relationships physicians, providers, and all colleagues. Collaborates with the leadership team in creating and sustaining a culture of compassion and healing. Promotes the Skyway culture by demonstrating genuine concern and compassion for all staff such that they may do the same for one another and our communities, patients and customers. Supports the organization's mission, vision, values and strategic initiatives. Develops, implements, and improves processes to ensure that established policies, goals, and objectives are achieved. What are we looking for? 2 years or more of experience in a behavioral health setting Undergraduate college degree or relevant experience required; Master's degree preferred Exceptional level of organization, reliability and independence A problem-solving orientation and eagerness to identify process gaps and implement practical solutions Ability to manage multiple priorities and respond with confidence and resolve during challenging or chaotic situations Excellent interpersonal skills Someone who embodies Skyway values: We embrace the whole person.
