Job Description:

A nonprofit services organization in New York City is currently seeking a Spanish-speaking Case Manager for a promising opportunity with their growing team in the Bronx.About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 9am to 5pm (35hrs)

Responsibilities:The Case Manager will:

  • Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals
  • Conduct face to face contacts (home visits/office visits)
  • Provide crisis intervention to clients and their families as needed.
  • Maintain ongoing awareness of clients' medical conditions so that services are appropriate to changing needs
  • Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
  • Provide counseling and orientation regarding tenant's rights and responsibilities to maintain and retain housing
  • Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and provide transportation
  • Maintain and complete required documentation including all contacts made with the client and any contact on their behalf
  • Participate in discharge planning with hospital staff for a hospitalized clients
  • Provide escort services as needed to medical or mental health services
  • Provide support and education to the client on budgeting to meet financial obligations
  • Perform other duties, as needed

Qualifications:

  • 1+ year of work HRS / HASA direct work experience
  • Bachelor's Degree
  • Bilingual (English and Spanish)
  • Extensive knowledge of Case Management principles & practices
  • Individual, Family and Group Counseling experience
  • Experience with Mental Health illness affected populations and/or Substance Use Disorder individuals
  • Knowledge of at least one of the following service areas: homeless related issues, criminal justice, HIV/ AIDS, substance abuse.
  • Microsoft Office proficient

Desired Skills:

  • Experience using AWARDs & CARES documentation system
