Contract Care Coordinator (Case Management) Pay Rate: $40-$45 per hour + expenses (Pay rate dependent upon licensure and experience level) Schedule: Monday-Friday, 8:00 AM to 5:00 PM MST A National Managed Care Organization is seeking a qualified Care Coordinator for a contract position in New Mexico. This role involves visiting members in Bernalillo, Valencia, Sandoval in Counties to provide high-quality care coordination services. You must live in New Mexico Job Responsibilities
- Conduct clinical assessments of members within regulated timelines, identifying those eligible for case management based on clinical judgment and assessment findings.
- Develop and implement comprehensive case management plans in collaboration with members, caregivers, physicians, and other healthcare professionals.
- Perform regular outreach through telephonic, face-to-face, or home visits as needed.
- Monitor care plans for effectiveness, document interventions and goal achievements, and adjust plans as necessary.
- Maintain an active caseload and conduct ongoing member follow-ups to ensure continuity of care.
- Facilitate interdisciplinary care team meetings and collaborate informally with team members.
- Promote integration of behavioral health, long-term services, and supports for improved member outcomes.
- Utilize motivational interviewing techniques and clinical guidelines to support and encourage member well-being.
- Identify barriers to care, assist members in addressing them, and coordinate resources as needed.
Requirements Education
- Bachelor's or Master's Degree (preferably in social science, psychology, gerontology, public health, social work, or a related field).
Experience
- 1-2 years of experience in case management, disease management, managed care, or medical/behavioral health settings.
- Minimum of one year of care coordination experience is required.
Licensure and Certification
- Valid driver's license with a good driving record. Reliable transportation is required for travel within Lea, Eddy, and Chaves Counties.
- Licensure is not mandatory, but candidates with the following certifications/licensures are preferred:
- Licensed Clinical Social Worker (LCSW)
- Advanced Practice Social Worker (APSW)
- Certified Case Manager (CCM)
- Certified in Health Education and Promotion (CHEP)
- Licensed Professional Counselor (LPC/LPCC)
- Respiratory Therapist
- Licensed Marriage and Family Therapist (LMFT)
Skills and Abilities
- Strong knowledge of case management practices, care coordination, and psychosocial wellness.
- Ability to perform clinical assessments and create effective care plans.
- Proficient in motivational interviewing techniques.
- Excellent organizational, communication, and time-management skills.
- Competent in promoting member-focused services that enhance continuity of care.
Preferred Qualifications
- 1-2 years of experience in behavioral health or managed care settings.
- Additional experience with asthma and depression disease management programs is a plus.
State-Specific Requirements Candidates must reside in New Mexico and hold a Bachelor's Degree or have at least one year of care coordination experience.
