Fitness ProgramPet InsuranceLegal AssistanceOptum Financial Service through ConnectYourCareBenefit Advocacy Center through GallagherSalary Compensation Range: $40,000-55,000MAJOR FUNCTIONS:The Case Manager is responsible for mental health and substance abuse counseling, intervention services, education services and concrete services. He/she works directly with residents in individual and group sessions on issues surrounding mental illness and chemical abuse/dependency and advocates a drug-free and medication compliant approach. The Case Manager is an integral part of the Clinical Team. He/she is directly supervised by the Program Director or Clinical Director.SPECIFIC DUTIES & RESPONSIBILITIES:Prepare treatment plans for residents diagnosed with severe and persistent mental illness.Responsible for ongoing resident discharge planning.Provide regular individual case management and group counseling sessions.Perform apartment visits and address resident's needs in relation to successful completion of Activities of Daily Living (ADL), including money management, medication management, personal hygiene, household activities and meal planning.Orient residents to the available supportive services, rules and regulations, and answer questions.Maintain up-to-date resident charts including treatment plans and case notes; develop and maintain good charting skills.Conduct groups to include Dual Diagnoses, Symptoms Management, Anger Management, and Money Management.Conduct education/prevention group forums to address topics of concern regarding mental health/chemical dependency and other related issues.Establish community and agency affiliations to strengthen the delivery of services for residents, including detoxification or hospitalization as needed.Provide therapeutic interventions to family members, as appropriate, in regards to resident's mental health/substance abuse issues.Enhance treatment services by supervising and facilitating group seminars, discussions and therapy sessions.Attend case conferences on site/in office (Park Avenue is an elevator building).Participate in QA/UR and Incident Reviews.If needed, assist in screening for contraband.Meet bi-weekly for supervision with Clinical Coordinator.Attend all required in-service training seminars.Dine with the community in order to set a relaxed tone and social environment by role modeling and monitoring activities of daily living related to proper dinning; supervise individualized specified dietary restrictions for those living with medical conditions such as diabetes and hypertension.Other relevant duties as assigned.REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIESBachelor's Level Clinical Degree and one (1) year of experience working in a clinical, mental heath, housing or chemical dependency setting preferred orHigh School Diploma/GED and three (3) years experience working in a clinical, mental health, housing or chemical dependency setting required.Candidate should be in pursuit of a professional credential that will ensure their status as a QMHP.Must be able to work a flexible schedule.Odyssey House is an equal opportunity employer maintaining a non-discriminatory policy on hiring of its personnel.
