NAFI NY provides innovative community-based services to children and families facing challenges across Orange County. The Parenting Love and Limits (PLL) program is seeking a compassionate Case Manager to provide case management services to families in this area. The PLL Case Manager plays a crucial role in supporting families to achieve their goals, addressing needs that may lead to Department of Social Services (DSS) involvement, and ensuring the safety and well-being of children and families.The Case Manager delivers services in the family's home and community to engage effectively within their natural environment. Responsibilities include supporting families, collaborating with community providers, connecting families to services, and ensuring overall safety and well-being.The PLL program, an evidence-based practice, aims to reduce residential stays and prevent residential placements for teens. Clients and their families participate in a six-week group and a minimum of eight to twelve family therapy sessions. The Case Manager works closely with the NAFI PLL clinician to address needs and support goal attainment.Key responsibilities include:Completing intake screenings and pre/post assessments according to NAFI NY and PLL standards.Facilitating PLL groups with the PLL clinician.Providing respite support activities for youth on the caseload.Assisting families in achieving service and treatment plan goals.Linking families to community services and providing support at convenient times, including evenings and weekends.Conducting required visits with families and completing model adherence and fidelity measures.Attending scheduled consultations and incorporating feedback.Collaborating with the Department of Social Services and connecting families with service providers.Assisting with crisis management and documenting progress notes, reports, and treatment/service plans.Reporting progress and barriers to stakeholders and courts.Qualifications to be a successful Case Manager: Bachelor's degree in social work, counseling, or a related field.Two years of related experience or training.Experience with troubled or delinquent adolescents preferred.Valid driver's license and own vehicleWillingness to travel within Orange County and other locations as needed.Experience in child welfare preferred.Benefits We Offer: 3-6 weeks' Vacation, 7 Sick days and 11 paid holidays each yearPet InsuranceCompany matching retirement plan and no cost retirement consultingNo cost Employee Assistance ProgramCompany paid for Life Insurance and Long-Term DisabilityPublic Service Loan ForgivenessHealth and Wellness: Comprehensive medical, dental, and other benefits to keep you and your family healthy and happy.
