To learn more about Swords to Plowshares, please visit our website at www.swords-to-plowshares.org.Job Title: Case Manager COVER ProgramSummaryThis is an in-custody and out of custody program position located both at the San Bruno Jail and Swords office 1060 Howard Street. The Case Manager supports the COVER Program by providing direct service to in-custody and out of custody veterans with relationship-building, assessment, counseling, crisis intervention, information and post-release referrals, mental health services, substance abuse intervention, benefits advocacy, life skills group facilitation, and other. The Case Manager delivers current information on community resources, including housing, residential and outpatient treatment for mental health and substance abuse. Essential Responsibilities Must be able to communicate effectively with a variety of personalities and be comfortable embracing the agency's Diversity, Equity, and Inclusion Initiative and the principles of Cultural Humility.Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered.Adhere to laws regarding confidentiality and reporting requirements.Conduct intake and enrollment forms to gather information to develop service and treatment goals.Perform verifications for justice-involved identified Veterans. Provide supportive counseling and referrals for transition out of custody through the VA Veterans Justice Outreach Program, Jail Behavioral Health Services (JBHS), and community partners. Maintain program participant files and online databases in an orderly and uniform manner in compliance with HIPAA, Department of Public Health (DPH), San Francisco Sheriff's Office (SFSO) and all other contractual requirements. Assists Administrative Assistant with compiling the COVER waitlist, responding to action requests and collecting the Military Service List and Housing Sheets. Documents all releases, transfers and program placements for contract reporting purposes. Perform follow-ups and track all post-release program participants and their housing, income, benefits, employment, probation and parole statuses. Participate in weekly case conferences with COVER and SFSO staff for case status updates, case closures and referrals to community linkages and/or warm handoffs. Liaison with San Francisco Veterans Justice Court (VJC) Requirements Proficiency in PC and Microsoft systems: Outlook, Excel, Word, etc.Ability to effectively use photocopiers, scanners, facsimile machines and voice mail systems.Excellent customer service, organizational and time management skills Strong written and oral communication skills Ability to work in a team environment. Excellent organizational and time management skills Flexibility and willingness to perform multiple tasks in a work environment with changing priorities. Proactive and take initiative in problem solving. Reliably detail-orientated Demonstrated ability to interact well with people from diverse backgrounds. Ability to follow through on assigned tasks. Must be able to obtain San Francisco Sheriff's Office Jail Clearance Must adhere to all jail rules and regulations. Qualifications Administrative support experience Flexibility and willingness to perform multiple tasks with changing priorities. Proactive problem solver with the ability to follow through on assigned tasks.Demonstrated ability to interact and communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agency's Cultural Humility Initiative Desired Qualifications Understanding of the functions, rules and regulations of a paramilitary organization Familiarity with San Francisco community-based organizations and resources Experience working with the in-custody/post-release population. Bilingual EducationA minimum of two years' experience providing mental health/substance abuse and social services to low-income and at-risk populations BA (Bachelor of Art) in Behavioral Science (Social Work, Forensic Psychology, Community Studies, etc.) This position requires prolonged sitting and repetitive hand movement using a computer keyboard in the performance of daily activities Must be able to perform all essential functions as described within what would be considered reasonable accommodations Must be able to communicate effectively with a variety of personalities and be comfortable with working with an at-risk client population, including embracing the agency's Cultural Humility Initiative The ability to provide a TB Test or Chest X-Ray within 60 Days of start date. Wage and Benefits Information: Wage range: This is a Full Time, Non-Exempt position with a starting wage of $26.00 to $30.00 per hourCompensation is based on a competitive public interest salary scale.
