Job Description:

Title: Case ManagerReports to: Team Leader Department of Corrections ProgramPurpose: Provides comprehensive case management to homeless clients being released from the Rhode Island Department of Corrections and current shelter clients determined to have significant incarceration histories for sex offenses and higher acuity, including assessment, case planning, evaluation of services and transition to housing.Duties and Responsibilities:Advocates for the mission of the organization and promotes the vision and direction of the agency's valuesDelivers services with safety, respect, and effectivenessFollows all policies and procedures of the organizationParticipates in assessment review with Team Leader and Diversion & Assessment SpecialistCompletes SPDAT assessments as needed with clientsWorks with Housing Locator to identify appropriate housing and assist with all move-in activitiesCompletes safety plan and risk minimization plan to appropriately support residents in community, including home visits and accompaniment to appointmentsWorks with Parole and Probation departments and other service providers specifically focused on supporting sex offenders in transition to community livingMaintains professional rapport with service user, peers, and other community service providersPractices fidelity to the Intensive Case Management Housing First Model, working with service users to achieve greater independence over timeProvides referrals to appropriate other service providers within the agency and externallyBalances the needs of caseload based upon acuity, length of time in the program, state of housing stability, etc., supporting 18-25 individuals at one timeAdvocates for the resident as neededReviews and evaluates resident progress on an ongoing basis and adjusts case plans as appropriateOrganizes and maintains resident case files in a timely fashion and in accordance with Crossroads policies and professional practicePerforms other duties as assignedRequirementsEducation and Experience:Bachelor's degree in Human Services field ORAssociates Degree in a Human Services field plus a minimum of 2 years experience in a Human Services field such as advocacy or case management ORA certification in Case Management plus a minimum of 2 years experience in a Human Services field such as advocacy or case management ORA minimum of 8 years experience in a Human Services field such as advocacy or case management.Knowledge, Skills and Abilities:Ability to work in a fast paced environmentMust work well under pressureMust be flexible and able to multi-taskMust be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, and/or, other conditions or situations that have impacted housing stabilityMust be solution-focused and organized in achieving objectives with all service usersMust be able to motivate and organize self to complete tasks with service users, often in non-therapeutic environmentsAbility to maintain confidentiality in all aspects of the work environmentExcellent oral and written communication skillsComputer skills and data entry requiredRequires reliable transportation outside of the public transit systemPhysical Demands and work environment: The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
