Metropolitan is Illinois' first comprehensive human services agency and reaches more than 122,900 individuals and families in Chicago, Evanston/Skokie, the southwest suburbs and DuPage County with services promoting education, economic stability, emotional wellness and empowerment.We are now hiring for a full-time Case Manager to join our Calumet Extended Family Support Team Team!ESSENTIAL JOB FUNCTIONS:Using intermediate level skills, conducts comprehensive client assessments as identified by the contract. Evaluates problem and service needs.Develops a written service plan with the client to meet client need, specifying goals to be achieved and services to be pursued.Supplements service plan to meet client needs.Provides, as appropriate, advocacy, case management, and support to client in coping with environment and interpersonal relationship issues.Educate and assist clients with navigating the probate court system. Budget management related to client financial assistanceUses expanded knowledge of resources and helps clients utilize them. Works collaboratively with community resources.Subsequent to consultation with supervisor, handles emergency and crisis situations within target population.Works within contract, grant, and program requirements including meeting timelines and reporting requirements.Reassess and monitors client situation and adequacy of service plan.Collaborates and participates with other program staff in the planning and implementation of service plans.Identifies systems issues and reports to program supervisor as appropriate. May work with community groups.With supervisory approval, authorizes limited amounts of financial assistance to own clients.Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.Works effectively with diverse staff and service populations. QUALIFICATIONS: Bachelor's Degree in Social Work or related field from an accredited college or university required.One year of case management experience in a social service, legal or healthcare setting required.System knowledge related to child welfare system preferred.Bilingual Spanish preferred.Strong interpersonal and time management skills. Must have access to a vehicle, valid driver's license and proof of insurance to perform essential job functions.Ability to maintain assigned community and home-based services. Ability to work effectively with diverse individuals and groups.Works effectively with diverse staff and service population.Staff may need to transport clients as needed.PHYSICAL DEMANDS:While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.The employee may occasionally lift and/or move up to 25 pounds.SALARY:Compensation is commensurate with years of related experience, position requirements, and candidate qualifications.
