SCOPE OF POSITION:The REMOTE temporary/contract Case Manager coordinates services of a grant-funded pilot program to delay institutional care to meet emotional, social, and psychological needs of grant participants. The Case Manager works closely with participants, families, and facility staff to provide support, guidance, and resources to enhance the overall well-being of older adults in Rockingham County. We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. The temporary/contract Case Manager conducts thorough assessments of participants' physical, emotional and social needs to develop personalized care plans. Further, the Case Manager collaborates and maintains a relationship with an interdisciplinary team, including medical professionals, therapists, caregivers, and service providers, to ensure seamless/integrated care delivery and further, facilitates access to community resources, support services, and educational opportunities that enhance participants' quality of life. This position serves as the primary point of contact for participants and their families, addressing concerns, answering questions, and advocating for their well-being. The Case Manager assesses participants' progress, gathers feedback, adjusts care plans/ensures care transitions are smooth and well-coordinated, and maintains accurate and up-to-date records in compliance with the program standards. REQUIRED EDUCATION AND EXPERIENCE
  • Completion of an Associate's or Bachelor's Degree in Nursing, Social Work, Public Health, Psychology, or related field; and
  • Proven experience in case management or advocacy work, preferably in a senior living or healthcare setting.
  • Valid, unencumbered driver's license in good standing, with acceptable driving record.
