Together, we are committed to breaking the cycle of homelessness and helping our clients achieve long-term self-sufficiency.Position Summary: LifeMoves | Guadalupe EIH is part of the overall SJ Bridge program, which includes the Goodwill of Silicon Valley - SJ Bridge employment program. LifeMoves will provideappropriate on-site services or connection to suitable services to include housing search for suitable housing solutions and provide basic needs. LifeMoves will provide support services that support self-sufficiency (e.g. financial workshops, resume building, nutrition, and conflict resolution) to participants with the goal of securing permanent housing for the participants. We provide opportunities that build brighter futures for the next generation.Case Manager Duties:Develops, implements and revises individual case plans and consistently monitors progress toward client goalsProvide on-going supportive and/or case management functions in accordance with the case/treatment planAct as professional liaison for LifeMoves and our community partnersCommunicate appropriately as needed with probation officers, social workers and family membersParticipates actively as a team member in supporting clients and the facility as neededStay current and knowledgeable about external community resources, referrals and service linkages Provides crisis management for clients; makes linkages for interventions as appropriateFacilitate at least one clinical or life skills group in addition to morning groupMaintains client and program records in accordance with applicable standards and regulations, grant requirements, etc.Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional statureParticipates in continuing education activities, remaining knowledgeable in area(s) of expertiseAttends weekly case consultation meetings and monthly trainings; meet regularly with supervisor to exchange pertinent information and receive supervisionOther duties as assigned by supervisorPosition QualificationsBachelor's degree in social work, psychology or related field, with a minimum oftwo years' case management experience Prefer minimum of 2 years' experience in chemical dependency or other drug treatmentsExcellent communication and interpersonal skills; proven ability to collaborate with community partnersUnderstanding of the issues faced by homeless individuals and other marginalized populations in crisis Availability to work occasional evenings and weekends.
