JOB SUMMARY:Under the general supervision of the Program Director, and the guidelines provided by managed care organizations, NYSOMH, NYSDOH and NYSOASAS, HCBS Case Managers provide services to persons with Serious Mental Illness (SMI). These services are offered in the setting best suited for desired outcomes, including home, or other community-based setting in compliance with Medicaid regulations. The setting may include programs that are peer driven/operated or peer informed and that provide opportunities for drop-in. Services may be provided individually or in a group setting and utilize (with documentation) evidence-based rehabilitation and recovery. The HCBS Case Manager assists assigned participating individuals in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services.ESSENTIAL TASKS:To perform this job successfully, an individual must be able to perform each essential duty listed satisfactorily. The HCBS Case Manager will provide the following services to assigned clients based on the plan of care:1. Instruction in accessing and using community resources2. Instruction in developing or maintaining financial stability and security (e.g., understanding budgets, managing money, and the right to manage their own money).3. Skill training and hands-on assistance of instrumental activities of daily living,4. Habilitation providing onsite modeling, training, and/or supervision to assist the participant in developing maximum independent functioning in community living activities.5. Facilitation and coordination of family support services as applicable6. Housing preservation and advocacy training, including assistance with developing positive landlord-tenant relationships, and accessing appropriate legal aid services if needed including skills to successfully live with roommates7. Assistance with increasing social opportunities and developing social support skills that ameliorate life stressors8. Instruction in self-advocacy skills including activities designed to facilitate participants' ability to access social service systems Instruction in developing strategies to manage trauma induced behaviors and/or PTSD as per a Trauma Informed Assessment1. - Assures that quality services, identified in the plan of care are delivered in a costconsciousmanner.
- Assures the recipient's satisfaction with the services provided.
- Collects data and documents the progress of the recipient in the case record.
- Makes any necessary revisions to the HCBS service plan
5. Provides Counseling and Exit Planning
- Assures that the recipient obtains, on an ongoing basis, the maximum benefit from the services received.
- Facilitates the development of support groups for the recipient, the recipient's family and informal providers of services.
- Mediates among the recipient, the family network and/or other informal providers of services when problems with service provision occur.
- Prepares the recipient for discharge from or admission to facilities or other programs.
6. Participates in individual and group supervision, treatment team meetings, case conferences, in-service training and other staff development activities as assigned.7. Observes all applicable ICL, governmental, and accrediting agency rules and laws regarding clinical records, confidentiality, and rights of consumers.8. Observes all applicable rules, procedures or laws regarding the reporting of incidents, including the reporting of alleged child abuse and neglect.9. - A high school diploma/GED and three years of experience in providing direct services toindividuals with mental illness or to homeless individuals.
