JOB TITLE:Case Manager EMPLOYMENT TYPE:Full-time, 40 hours/week SCHEDULED HOURS:Monday-Friday 8AM-4:30PM, with some flexibility PROGRAM/LOCATION:Homeless to Housing, Manchester, CT PC#:2785 ABOUT THE PROGRAM: Homeless to Housing Conducts outreach, mental health & substance abuse services and case management to people experiencing serious mental illnesses-including those with co-occurring substance use disorders-who are homelessness. Participants must be homeless as defined by living in a shelter or living unsheltered or places not fit for human habitation. POSITION HIGHLIGHTS: We are seeking a qualified Bilingual Spanish candidate to provide outreach to unsheltered individuals and those in homeless shelters who are experiencing serious mental illness. Key responsibilities include:
  • Assisting individuals in navigating systems and securing affordable housing
  • Helping clients receive/maintain entitlements and benefits
  • Connecting clients with appropriate mental health treatment
  • Coordinating care with collateral providers
  • Supporting clients with money management and daily living activities

  • Complete outreach to homeless sites and shelters
  • Establishes rapport and working relationship with assigned clients.
  • Develops and writes comprehensive client service plans in conjunction with client.
  • Assesses and involves natural supports in the treatment process, when possible.
  • Demonstrates competency in age related or specialty issues and developmental needs for each population served.
  • May provide assistance with daily living, as indicated for population served.
  • Assists in obtaining benefits, housing, treatment, legal services, medical services, education and employment, as needed.
  • Assists with money management and representative payee, as needed, for population served.
  • Schedules and provides transportation to and from appointments, as needed.
  • Assists with meal preparation, house maintenance and general shopping as appropriate and necessary for population served.
  • Provides crisis intervention and support as clinically indicated.
  • Meets regularly with each client to assess current status and progress.
  • Use SOAR Model to help clients obtain social security.
  • Formulates adequate discharge plans for all cases terminated from assigned caseload.
Maintains Family and Other Collateral Contacts to Ensure Quality, Client-Centered Services:
  • Works as liaison between agency and state and private hospitals, other community- based services, family members and significant others.
  • Assists in linking client with community support systems and agencies.
  • Advocates for clients' rights and access to services.
  • Provides overall case coordination and management.
Maintains Documentation and Other Methods of Communication to Maintain Quality Services:
  • Compliant with recordkeeping requirements consistent with program and agency standards including the monitoring and charting of clients' behavior and progress toward specific goals.
  • Completes all required documentation and statistical reports related to client service in accordance with established agency procedure.
  • Attends and participates in weekly staff meetings, case conference and supervision.
Education:Bachelor's Degree in human services or related field Experience: 2+ years working with identified populations and experience conducting homeless outreach and knowledge of homeless engagement. Licensure/Certification/Registration: Valid driver's license required. CPR/First-Aid Certification within 6 months of hire. Additional: Bilingual (Spanish speaking) preferred COMPENSATION*: $20.65/hour minimum, and up commensurate upon experience *Actual rates are determined at the time of offer and are based on relevant experience, as well as, adherence to our internal equity policy.
