Description
Comunilife is looking for experienced Case Managers to fill open positions for their Dona Rosita I Program in the Bronx. The Case Manager is responsible to assess the resident's needs and develop case plan to ensure that resident is linked to needed services both inside the residence and in the community. Assist resident in developing necessary skills to live more independently in permanent housing. Responsible for general case management, working with residents families and significant others for Residence, which provides congregate treatment services, i.e., one (1) meal a day; case management and recreational activities to persons living with HIV/AIDS. Other responsibilities include working harmoniously with Dona Rosita Program staff in accordance with Comunilife principles and guidelines. Essential Duties
+ Coordinate program admission process with HASA Case Management Unit and Housing Unit.
+ Conduct interview of referrals within 24 hours of the HASA Referral.
+ Follow HASA procedures regarding the referral process and the completion of forms and statistical records.
+ Arrange and coordinate the move of accepted residents as determined by HASA and the program.
+ Monitor and assist the residents with problems at local entitlement offices. (Social Security, Medicaid, income support, etc.)
+ Establish permanent housing referral network.
+ Implement and monitor the service needs of residents in order to provide case management services which link residents with other agencies, health providers and support systems.
+ Institute appropriate referrals for services and functions as a liaison/advocate to ensure receipt of required services.
+ Facilitate individual and group counseling to residents, i.e., substance abuse, money management, relationships with family and loved ones current information on HIV/AIDS, treatment and drug trial programs.
+ Provide monthly caseload update to Clinical Supervisor. Document all direct activities including resident served case closures and direct service advocacy interventions.
+ Document all contacts and referral activity in resident records.
+ When necessary and appropriate, accompany residents in medical appointments and/or arrange ambulate services.
+ Attend all mandated meetings and in-service training.
+ Assist resident in gaining the skills necessary to live more independently and arrange for placement in a permanent housing program.
+ Establish working relationship with HASA case management staff to ensure quality client care.
+ Participate in ongoing evaluation process to assess direct service activities for program modification. Future planning and to assist in the preparation of required reports.
+ Provide in-service training to other staff members as determined by Program Director.
Qualifications
Education and Experience Requirement(s)
Bachelors Degree required; with minimum two years field work experience and direct mental illness work experience. Associates degree with commensurate experience in case management. Individual, family and group counseling experience required, particularly with mentally ill populations. Preference given to persons with experience in working with homeless individuals in Congregate Programs, bi-lingual, bi-cultural in Spanish, a plus.
