We deliver and coordinate a continuum of high-quality healthcare with the goal of improving the health of our communities.Job SummaryThe Case Manager in the Behavioral Health department is responsible for providing a range of services for clients with either psychiatric and/or substance use issues in both the inpatient and ambulatory setting in order to ensure that patients' needs are met. Acts as a liaison and advocate for patients by ensuring necessary appointments are scheduled, all necessary documentation is provided to care providers, coordination of transportation as needed, and connecting patients to community resources. The Case Manager is the subject matter expert for patients on available community resources for each individual patient's needs.
