Title: Case Manager
Schedule: Full Time
Salary: $58,000 - $63,000/year based on experience

We are currently seeking a dedicated and passionate Case Manager with expierence in Senior Living.
Case Manager Responsibilities include:
Orient and assist each resident to adjust to life in the community
Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to facility, including their social, emotional, and psychological needs
Assist each resident to maintain family and community ties and to develop new ones
Complete quality management reviews to ascertain if the department is meeting state regulations and PSL policies and follow up as needed
Work with the care department and others to promote and protect resident rights and the psychological well-being of each resident
Case Manager Education & Experience:
Bachelors degree with major course work in human services AND one year of full-time work experience in a related field OR
Associates degree with major course work in human services AND three years of full-time work experience in a related field required
Case management experience required
Memory Care/ Assisted Living expierence required
Compassion for and desire to work with the elderly
