Case Manager

A social services organization in Manhattan that offers comprehensive services to individuals affected by HIV/AIDS is seeking Case Managers. The Case Manager will be responsible for advocating for clients, conducting intakes and assessments, and making referrals for other services.

The base pay range for this position is $22 to $25 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. Responsibilities:

• Manage a full-time case load
• Prepares intake documents, service plans and assessments/reassessments for clients and their families
• Create a service plan that includes well developed goals and progress markers
• Ensure all client documentation is completed on time, accurate, and compliant with the contract provider s requirements.
• Provide quality case management services and crisis intervention
• Attends and actively participates in weekly team meetings and/or case conferences

Qualifications:

• Bachelors in Social work or similar field
• Two years of case management experience in supportive housing/housing
