Job Description:
Assesses the needs of individuals with HIV disease and assists with linking them to necessary services/resources related to their medical status and psychosocial needs. Provides necessary monitoring, follow-up, and coordination of services, including the provision of treatment adherence. The case manager, along with the patient, will assist in creating support systems and resources to fulfill unmet needs. Assists patients with accessing medical insurance, Social Security benefits, or Pennsylvania's Special Pharmaceutical Benefits Program (SPBP). Must be available to travel to the patient's home, to the hospital or to another agency or community center if needed. Must also be available to accompany patients to appointments to offer guidance and support. Education Bachelors Degree in Social Work (Required) Masters Degree in Social Work or related field (Preferred) Experience 5 years experience working with HIV client services (Required) Experience in mental health/ intellectual diagnosis, addiction, case management and client advocacy (Required) Bilingual in Spanish (Required) Licenses & Certifications Driver Drivers License (Required) Your Tomorrow is Here!
