Job Title: Case ManagerJob Summary: Case Managers provide core case management services, including comprehensive care management, care coordination and health promotion, encouraging and assisting eligible clients to enroll in the Medicaid/health insurance plan of their choice, assist with housing needs, and provide participant and family support and referrals to community and social supports as needed. They conduct screenings and assessments for high-risk needs, conduct monitoring, and follow-up services.Shift Hours: This position is full-time Monday through Friday from 9:00 am to 5:00 pmSalary: The salary range for the role is $45,000 - $55,000 per year.Location Address: 2090 Adam Clayton Powell Jr Boulevard, New York, NY 10027. Our office is easily accessible by public transportation.Workplace Flexibility: This position is Flexible: This position has flexible work flexibility where for which staff have flexibility to complete their work day from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.What you will be doing:
  • Provide targeted case management as a member of a multidisciplinary care team to clients enrolled in NCCBHC using team-based approach to treatment and services working collaboratively with therapists, psychiatrists/PNPs, peer specialist, substance use specialists, nurses, clients and family members
  • Engage clients in a collaborative relationship, empowering them to self-manage their mental health, substance use, physical, and psychosocial needs to improve and maintain well-being
  • Screen and assess clients for social drivers of health (SDOH), housing instability, food insecurity, transportation needs, utility difficulties, interpersonal safety, education and employment needs
  • Engage clients with urgent and crisis needs at all stages of NCCBHC services to provide support to resolve crisis, and provide targeted case management expertise for person-centered evaluation and treatment planning
  • Support clients to access supported education and supported employment services
  • Provide core care management services, including care coordination and healthy promotion, comprehensive transitional care, and referral to community and social supports
  • Train clients on relevant chronic diseases, preventive care, medication management (medication reconciliation and adherence)
  • Provide care management services in a person/family-centered, strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner
  • Support clients with aftercare upon discharge from ERs, detox, inpatient hospital services
  • Conduct community outreach and engagement to support client to retain in NCCBHC services
  • Support clients to access housing, income supports (entitlements), and other needs
  • Contribute to ongoing organizational commitments to diversity, equity, inclusion, and antiracist practices in delivery of services and work as member of the NCCBHC
  • Conduct monitoring and follow-up services to promote continued engagement in referred rehabilitative and treatment services
What we are looking for:
  • New York Certified Peer Specialist (NYCPS), Credentialed Family Peer Advocate (FPA-C) or Credentialed Youth Peer Advocate (YPA-C) with 3 years of work experience AND/OR
  • Bachelor's Degree in human services, social work, psychology, rehabilitation and related field
  • 2-years experience working with people with serious mental illness/substance use disorders
  • Individuals with lived experience in the areas of mental health, substance use, or criminal legal are encouraged to apply
  • Bilingual Spanish-speaking preferred
